Research & Development: Palliative Carecaring hands

  • Coordinated Pathways:
    Supporting Carers and Families in a Palliative Approach for Care
    Funded through the Local Palliative Care Grants Program, Department of Health and Ageing

    The aim of this project is to build the family carer’s capacity to enable older people with dementia to stay at home until they die if this is their choice. This included developing information for carers (based on their input) and developing and implementing training for staff to increase their skills and confidence in providing support at end stage dementia care, and to discuss issues such as advance care planning, end stage dementia, and dying at home.
