In Home Care

Choosing flowersSupport for older people who want to stay living at home, provided in your home by trained Resthaven home support workers. You may be living with your spouse, family or on your own.

Your Resthaven Coordinator will visit you at home and work with you, and your family and friends if you wish, to tailor a plan that meets your needs.

You may have support with household tasks, home maintenance, shopping, social activities, keeping healthy, personal care and nursing care. Support ranges from one hour a week for people with lower care needs to more complex planned and coordinated packages of care for people with high support needs.

Low Level In-home Support (funded by Home and Community Care - HACC)

Feeding the chickensThese services are designed for people with low care needs who could do with some help. Individually tailored support, usually 1-3 hours a week, may include but is not limited to:

  • household tasks such as cleaning, laundry, meal preparation and gardening
  • home maintenance
  • shopping
  • emotional support
  • support to keep in touch with family, friends and the community
  • assistance with personal care
  • support with medication.

Resthaven provides Low Level In-home Support funded by HACC in northern, eastern and southern metropolitan Adelaide, as well as the Limestone Coast.

Community Aged Care Packages (CACP)

These services are designed for people with more complex care needs

Individually tailored support may include, but is not limited to:

  • household tasks such as cleaning, laundry, meal preparation and gardening,
  • home maintenance
  • shopping
  • emotional support
  • support to keep in touch with family, friends and the community
  • assistance with personal care such as showering and dressing
  • support with medication
  • attending medical appointments
  • other assistance as negotiated according to needs.

Resthaven provides Community Aged Care Packages in northern, eastern, western and southern metropolitan Adelaide, Adelaide Hills, Murray Bridge and Mallee, Riverland and Limestone Coast.

Extended Aged Care at Home (EACH) packages

These services are designed for people with complex high care needs.

Support is negotiated according to each person’s individual circumstances and may include, but is not limited to:

  • household tasks such as cleaning, laundry, meal preparation and gardening
  • assistance with personal care such as showering and dressing
  • professional nursing and other allied health services
  • support to keep in touch with family, friends and the community
  • support with medication
  • continence support and advice
  • attending medical appointments
  • home maintenance
  • shopping
  • emotional support
  • other assistance as negotiated according to needs.

Resthaven provides EACH packages in northern, eastern, western and southern metropolitan Adelaide, Adelaide Hills, Murray Bridge and Mallee, Limestone Coast and Riverland areas.

Extended Aged Care at Home Dementia (EACHD) packages

These services are designed for older people who have been assessed as having behaviours of concern and psychological symptoms associated with dementia, which impact on their ability to live independently at home.

A Resthaven Coordinator works closely with the older person, their carer/family, and other service providers such as GPs, aged care providers or Alzheimer’s Australia, to develop a plan of support according to individual need.

Support may include but is not limited to:

  • coordination and case management
  • working with other specialist services to develop therapy/activities/strategies that assist in managing behaviours and symptoms related to dementia
  • assistance with personal care such as showering and dressing
  • professional nursing and other allied health services
  • support to keep in touch with family, friends and the community
  • attending medical appointments
  • support with medication
  • continence support and advice
  • household tasks such as cleaning, laundry, meal preparation and gardening
  • shopping
  • meal preparation
  • other assistance as negotiated according to needs.

Resthaven provides EACHD packages in northern, eastern, western and southern metropolitan Adelaide, Adelaide Hills, Murray Bridge and Mallee, Limestone Coast and Riverland areas.

Veterans' Home Care (VHC)

poppiesVeterans' Home Care (VHC) is a Department of Veterans' Affairs (DVA) program designed to assist eligible veterans and war widows or widowers who want to stay living at home, but who need some practical support.

Resthaven is an approved, contracted VHC service provider delivering this program on behalf of DVA.

Services provided include low level home support as determined by DVA assessment.

Referrals need to be made directly through the DVA.

Veterans’ Community Nursing

Veterans' Community Nursing is a Department of Veterans' Affairs (DVA) program designed to provide clinical nursing and/or personal care services to eligible veterans and war widows or widowers in their own home. DVA Community Nursing aims to restore or maintain the maximum level of health and independence and helps to avoid premature or inappropriate admittance to hospital or residential care.

Resthaven is an approved, contracted service provider delivering DVA Community Nursing on behalf of DVA.

A full range of in-home nursing services, including palliative care is available.

To see if you are eligible and to apply for VHC services,
call DVA: 133 254 or Country (toll free) 1800 555 254.

Referrals are welcome from general practitioners, hospitals, community nurses and allied health professionals. All referral agencies must identify their DVA provider number on the referral.

Transition Care Program (TCP)

The Transition Care Program (TCP) is designed to help you improve your independence and confidence after a hospital stay.

TCP provides services that may include domestic assistance, personal and social support, nursing and therapy services to support your recovery. This means that you and your family or carer have time to consider your long-term care and support needs. These services are provided in a residential aged care setting or in your home in a community setting.

Resthaven is an approved provider of TC services.

The hospital will make arrangements with Resthaven if you are eligible.

Resthaven provides TC services (community based, in your home) in the southern metropolitan Adelaide, Adelaide Hills, Murray Bridge and Mallee, Limestone Coast and Riverland areas.
