
Resthaven commissioned as Care Finders by the Primary Health Network
Resthaven is now providing services for older people at risk of homelessness through the ‘Care Finder’ Program.
Resthaven Executive Manager, Sue McKechnie, explains, ‘The Care Finder Program was announced as part of the Australian Government’s response to the Royal Commission into Aged Care Quality and Safety.’
‘Resthaven has been commissioned by both the Adelaide Primary Health Network (PHN) and the Country SA PHN to provide the care finder services, with an initial focus on older people who are at risk of homelessness, then expanding to a broader cohort in metropolitan Adelaide.’
‘The aim of Care Finder services is to deliver intensive support to help vulnerable older people, who may have difficulty accessing aged care information, to access and understand aged care, and connect with other community supports.’
‘Care Finders can:
- Support clients to interact with My Aged Care so they can be screened for eligibility and referred for assessment
- Explain and guide clients through the assessment process
- Assist clients to find the aged care supports and services they need and connect with other relevant supports in the community
- Undertake check-ins and provide follow-up support once services have commenced to make sure clients are still receiving services and needs are being met.’
‘The Care Finder services are provided as part of Resthaven’s Agedcare Alternatives Information and Service Navigation Centre, which opened in 2011.’
