    Home care packages

    Home Care Packages

    Resthaven supports older people who require assistance to remain at home through individually tailored support services called ‘Home Care Packages’.

    Services are delivered using a Consumer Directed Care (CDC) approach, which gives the older person choice and flexibility in the way their care and support is provided.

    DVA nursing

    Community Nursing

    Resthaven is contracted by the Department of Veterans’ Affairs (DVA) to provide a full range of nursing services, including palliative care, for eligible Veterans.

    Referrals are welcome from general practitioners, hospitals, community nurses and allied health professionals. All referral agencies must identify their DVA provider number on the referral. Read this brochure for more information, and to find out if you are eligible.
