Application for Residential Care
Resthaven offers a central admission service for all its residential aged care facilities. Our admissions team assists as much as possible to provide information and to ensure a smooth transition to residential care.
An independent assessment by the Federal Government Aged Care Assessment Team (ACAT) is required to determine if the applicant is eligible for low or high level residential care. A copy of this assessment is to be forwarded to Resthaven with an application for residential care.
Resthaven requires information about each applicant’s current circumstances, health, and financial situation. Under the Privacy Act 1988 (Cth) (as amended 2007), Resthaven also requires permission to collect personal information. Following approval, Resthaven conducts an assessment through our admissions process. Each applicant’s individual circumstances are considered to ensure that accommodation vacancies are appropriate for the applicant and are allocated to applicants with the greatest needs.
There may be a waiting list for accommodation, which is offered to people with the highest care needs, rather than those who have been on the waiting list the longest. It is important that information provided reflects applicants’ needs, and that the admissions department is advised of any changes in personal circumstances, as such changes may affect care needs or urgency of placement. Once the application has been processed and the applicant is on our waiting list, it is essential that the admissions department is notified monthly to maintain the application and update personal circumstances.
Tours of the low level care facilities are conducted regularly at each site. A tour of high care at each site can be arranged by individual appointment.

The Commonwealth Department of Health and Ageing publish a booklet with independent information regarding the process of moving into a residential aged care facility. Whilst an applicant is waiting for residential placement, respite or community services may be available to offer support at home.
