Eligibility/Fees

Eligibility
Eligibility guidelines for each program vary. For some programs a simple self-referral or a referral from a friend, family member or doctor is all that is required.

For Community Aged Care Packages (CACP), Extended Aged Care at Home (EACH) packages and Extended Aged Care at Home Dementia (EACHD) packages, an assessment by the Aged Care Assessment Team (ACAT) is needed.

To find out how to contact the nearest Aged Care Assessment Team, contact the Aged Care Information Line on Freecall™ 1800 500 853.

Referral by a doctor or allied health professional is required for professional nursing, and some allied health services. Contact the local community service office on 1300 13 66 33 to inquire about access to the most appropriate community service.

Once eligibility for services has been determined and referral accepted by the program, a place becomes available and a Coordinator or Allied Health Professional discusses specific needs and circumstances. The services available are explained in detail and a tailored plan to meet individual needs is developed.

Fees
Resthaven's services are primarily subsidised by funds from the Australian and South Australian Governments. There is a recommended fee structure for our services. People using our services are asked to contribute towards the cost. Our fee schedule is reviewed annually, and fees for packaged care (CACP, EACH, EACHD) are set by the government. Fees and mode of payment are discussed and agreed with each individual.
