    Regain Confidence and Independence

    Resthaven’s Restorative Care Pathway is a time-limited, goal-oriented service that supports older people living at home to recover after illness or injury, or to address early signs of functional decline. Over a maximum 16-week period, the program helps you improve your ability to manage daily tasks, regain strength, and continue living independently.

    Support Where You Need It

    Services can be delivered in your home or at a Resthaven Wellness Centre, depending on your needs and preferences.

    Our team will work closely with you to create a personalised care plan that reflects your goals, lifestyle, and choices.

    Support may include:

    • Assessment and therapy from allied health and nursing professionals
    • Home-based or group exercise programs
    • Guidance with daily activities and safe home living
    • Equipment and mobility aids
    • Nutritional advice and support
    • Confidence-building and community connection
    • Information on managing health conditions

    Eligibility and Referral

    You may be eligible if you:

    • Are slowing down physically or mentally
    • Need help with everyday activities
    • Wish to remain independent and avoid long-term care

    The Restorative Care Pathway is funded by the Support at Home program.

    To access Restorative Care, you’ll need an assessment by an Aged Care Assessment Team (ACAT).

    A Team-Based Approach

    The Restorative Care Pathway is provided by a multidisciplinary team, including your GP, allied health professionals, nurses, and care coordinators.

    Together, we focus on healthy, active ageing, to support quality of life, avoid unnecessary hospital stays, and delay or prevent the need for long-term care.
