    Resthaven’s Community Services Team deliver Home Support and Wellness services, offering practical help and personalised care to support older people to live independently at home.

    Services may include personal care, help around the home, transport, social support, nursing, allied health and more.

    To access government-funded home care services, you generally need to be:

    • Aged 65 or over (50 or over for Aboriginal and Torres Strait Islander people), and
    • Needing support to remain living independently at home
    We also offer specialised services to those who are:
    • Aged 65 years and over
    • Living with dementia 
    • Aboriginal and Torres Strait Islanders who are aged 50 years and over
    • Culturally and Linguistically Diverse (CALD)
    • Veterans of the Australian or allied defence force, including their spouse, widow or widower
    • Located in rural or remote areas
    • Financially or socially disadvantaged
    • Homeless, or at risk of becoming homeless
    • Care leavers (including Forgotten Australians, Former Child Migrants and Stolen Generations)
    • Parents separated from their children by forced adoption or removal
    • People from lesbian, gay, bisexual, trans/transgender and intersex (LGBTI) communities.

     

    Eligibility is determined through a formal assessment arranged by My Aged Care. Resthaven can help guide you through the process.

    Yes. If you need an interpreter, just let a Resthaven staff member know.

    We can arrange a qualified interpreter through the Translating and Interpreting Service (TIS) by calling 131 450.

    You can refer yourself, or someone can refer on your behalf — such as a family member, friend, GP or health professional.

    To access most government-funded home care services (including through Resthaven), you’ll need to register with My Aged Care.

    Resthaven also offers services on a fee-for-service basis through Lifestyle Choices Plus, without needing a My Aged Care referral

    My Aged Care (MAC) is the Australian Government’s starting point for accessing aged care services.

    To receive government-funded home care, contact My Aged Care at 1800 200 422 or visit myagedcare.gov.au.

    An independent assessment will determine your eligibility and support needs.

    All access to Australian Government-funded home support or home care services, apart from some services to Australian veterans, is channelled through My Aged Care. This includes all access to Resthaven’s Australian Government-funded services.

    If you’re not eligible, or prefer not to use government-funded services, we offer Lifestyle Choices Plus, a private service option with no waiting list.

    Fees vary depending on your income, the type of service, and the level of government funding.

    Some costs are set by the Australian Government, while others are set annually by Resthaven (e.g. for CHSP).

    Participants will be required to pay a contribution towards services under Independence Support and Everyday Living Support. No contribution is required for services under Clinical care. This amount is calculated by Centrelink, based on your income and is advised to you by Centrelink.

    Yes. When speaking with My Aged Care, you can nominate Resthaven as your preferred provider.

    When you make contact with MyAgedCare by phone on 1800 200 422, you will be asked questions to establish your eligibility for Australian Government-funded services, and you will be able to advise them of your preferred provider.

    The number of care hours you receive depends on your individual needs and funding level (Classification 1–8), as assessed through My Aged Care.

    Resthaven works with you to create a personalised care plan based on your preferences and goals. This includes how and when you’d like services delivered.

    Your funding is managed by Services Australia. A portion (10%) is automatically allocated to care management, which is an essential part of your support.

    Care management is considered a form of direct care under the Aged Care Act.

    It includes:

    • Coordinating care and services
    • Developing and reviewing your care plan and the outcomes of care and services
    • Coordinating and monitoring your care and services
    • Supervising and monitoring the staff who deliver services to ensure the quality of clinical care
    • Referrals to allied health or hospital care
    • Communicating with your GP, specialists, family or carers

    Care management ensures your services remain safe, high quality, and responsive to your changing needs.

    Resthaven offers home support services across metropolitan Adelaide, the Adelaide Hills, Murraylands, Riverland, lower Barossa and Gawler, the Limestone Coast and the Fleurieu Peninsula, so there may be a service available near you!

    Respite care supports carers in their caring role by supporting the person they care for with meaningful activities in a safe, supported environment.

    It helps carers rest and recharge while ensuring the person receiving care continues to be supported at home or in the community.

    We recognise that carers also need care and need to take a break from their caring role in order to manage other aspects of their lives and look after their own health and wellbeing, which enables the care recipient to remain living in the community longer.

    Respite care is provided in a number of settings to allow flexibility for carers and participants.

    Resthaven offers a range of respite options:

    • In-home respite
    • Centre-based day groups (small group)
    • Overnight cottage-style respite (1–4 people in a suburban house).
      This overnight accommodation is available for a minimum of one night but also for longer periods (e.g., up to 14 nights).
    • Short-term residential respite in aged care homes (up to 63 days per year, often taken in blocks of about two weeks or more).

    Restorative Care Pathway

    If you’re approved for Restorative Care funding through My Aged Care, Resthaven will work with you and your health team to set goals and create a short-term care plan.

    Services are delivered over 12 weeks, focusing on improving your independence and wellbeing.

    Commonwealth Home Support Programme (CHSP)

    CHSP offers entry-level support to help older people stay safe, independent, and connected at home and in the community.

    It’s available to people aged 65 and over (or 50+ for Aboriginal and Torres Strait Islander people) who need a small amount of help with everyday tasks.

    Services may include:

    • Domestic assistance
    • Social support (one-on-one or in groups, at home or in the community)
    • Shopping assistance
    • Health and wellness services such as podiatry, physiotherapy, occupational therapy, continence support and counselling

    The program also supports people experiencing hoarding or squalor situations that may pose a risk to their health or housing.

    Support at Home

    On November 1, 2025, Support at Home replaced Home Care Packages.

    Support at Home offers flexible, tailored services based on your individual needs, as assessed through My Aged Care. There are eight levels of funding available — higher levels are designed for people with more complex care needs.

    Veterans’ Care

    Home care and community nursing services for eligible Department of Veterans’ Affairs (DVA) clients.

    How to apply for aged care services

    Simply put, there are six steps:

    1. Contact My Aged Care

    Call 1800 200 422 or visit myagedcare.gov.au

    2. Provide your details

    Have your Medicare number, address, and contact details ready.

    3. Explain your needs

    My Aged Care will ask questions about your daily life and goals. You’ll receive a referral code—keep this handy!

    4. Get assessed

    A trained assessor will visit you at home to understand your needs. You can nominate Resthaven as your preferred provider.

    5. Await your referral

    After your assessment, you’ll receive a support plan outlining your approved services and funding level.

    6. We call you

    Once you’re approved, contact Resthaven with your referral code—or we’ll contact you if you listed us as your provider.
