What is Reablement?
Reablement is a short-term, goal-focused approach designed to help individuals maintain or regain their independence. It supports people to build confidence, improve everyday skills, and increase their ability to resume daily activities.
By focusing on what matters most to each person, reablement encourages active participation in setting and achieving personal goals. It is a key part of the Support at Home program and plays a central role in the Restorative Care Pathway, aiming to restore function, promote self-reliance, and reduce the need for ongoing support.
Therapy and Allied Health Services
Services are delivered in your home or at a Resthaven Community Services site, and may include:
- Physiotherapy – Improve movement, strength, and balance with personalised therapy plans.
- Occupational Therapy – Learn new ways to manage everyday tasks and maintain independence at home.
- Podiatry – Take care of your feet to stay mobile and safe.
- Speech Pathology – Support for communication or swallowing difficulties after a stroke or surgery.
- Nursing – Clinical nursing and rehabilitation support in your home.
- Dietetic Services – Nutrition and dietary advice for managing chronic health conditions.
- Counselling and Social Work – Emotional and social support tailored to your needs.
- Exercise Groups – Join group sessions to build strength, prevent falls, and stay active.
- Falls Prevention – Individual and group programs focused on improving balance and reducing risk.
- Chronic Condition Self-Management – Support for conditions such as diabetes, heart health, arthritis, Parkinson’s, or stroke.
- Continence Management – Strategies and support to maintain dignity and improve quality of life.
