    New aged care assessment process

    The Australian Government has introduced a new Single Assessment System making it easier for older people to access aged care and adapt services as their needs change.

    Previously, different assessment teams handled basic and complex care needs. This often meant repeating your story and being referred between organisations.

    Launched in December 2024, this system replaces the old two-step process with one streamlined process for all levels of care.

    What’s Changed?

    • One assessment process for all aged care services, instead of two
    • One national team to support you
    • Fairer and consistent access across Australia, including rural and remote areas
    • Easier to update your care plan if your needs change

    How it Works

    When you or a family member needs support, simply contact My Aged Care:

    A trained assessor (like a nurse or social worker) will arrange a home visit to talk through:

    • Your health and mobility and memory
    • Your emotional and social wellbeing
    • Your home environment
    • Your goals and preferences

    This relaxed conversation forms the basis of your aged care support plan. Family or carers are welcome to join.

    One Tool, One Workforce

    Assessors now use the new Integrated Assessment Tool (IAT) to help match services to your unique needs. The workforce has also been unified, so you’re supported by one national team — reducing duplication and improving continuity of care.

    Additionally, as of 1 July 2025, First Nations assessment organisations are now providing culturally safe and trauma-informed assessments to support older Aboriginal and Torres Strait Islander people on their aged care journey.

    Who is Eligible?

    You can access the Single Assessment System if you are:

    • 65 years or older
    • 50 years or older if you’re Aboriginal or Torres Strait Islander

    Assessments are fully funded by the Australian Government. There is no cost to you.

    What Happens After?

    Following your assessment, My Aged Care will guide you through available services based on your needs, such as:

    • Help at Home (cleaning, meals, transport)
    • Home Care Packages
    • Residential care
    • Respite and transitional care

    You can then choose a provider — like Resthaven — that suits your needs and location.


    Coming Soon: Support at Home Program

    From 1 November 2025, the new Support at Home program will offer more flexible and tailored home care services. If you already receive a Home Care Package, you may benefit from reduced out-of-pocket costs leading up to the transition.
